Why Screening is Important:

Whether you’re planning for the future, considering pregnancy, or simply want to better understand your reproductive health, a Fertility Wellness Screening at Midwest Fertility Center can provide helpful information about key fertility measures.

All Fertility Wellness Screening options are self-pay. Final payment is due at the time testing is performed, and insurance will not be billed for these services.

Endometriosis Treatment

Basic Fertility Wellness Screening

A starting point for understanding your reproductive health, individually or as a couple.

For Women: $80

  • Anti Müllerian Hormone (AMH) Blood Test – assessing your ovarian reserve
  • Basic Transvaginal Ultrasound – to evaluate the uterus and ovaries

Important: Ovarian-reserve screening estimates egg quantity, not egg quality, the ability to conceive naturally, or the chance of a live birth. It does not evaluate the fallopian tubes.

For Men: $50

  • Basic Semen Analysis

Important: This screening does not include sperm DNA fragmentation testing and does not replace a complete male infertility evaluation or consultation with a physician.

For Couples: $130

  • Includes both the women’s and men’s basic Fertility Wellness Screening services

Planning Your Visit:​

For the Basic Semen Analysis, plan for 2-3 days of abstinence before collection. Producing the sample at the medical office is required.

Advanced Female Fertility Wellness Screening

A broader fertility wellness picture.

For Women: $280

  • Fertility History Review – to review your menstrual, pregnancy, medical, and surgical history and review your results with a provider
  • Basic Transvaginal Ultrasound – to evaluate the uterus and ovaries
  • Ovarian Reserve Testing – includes FSH, LH, and estradiol
  • Thyroid and Prolactin Testing – includes TSH, free T4, and prolactin to screen selected hormonal factors (fasting is required for 12 hours prior to lab test)
  • Ovulation Assessment – includes one progesterone blood test
  • BBT and Vitamin Guidance – to provide instruction on basal body temperature chart (BBT) and recommended vitamins


Important: Does not include HSG, saline/sono-HSG, tubal-patency testing, genetic carrier screening, Pap testing, or a physician consultation. Screening cannot identify every cause of infertility or guarantee future fertility.

Planning Your Visit:

This screening requires two appointments. Please fast for 12 hours prior to your first appointment. Your second appointment will be scheduled at the end of your first visit.
